The position is responsible for conducting needs assessments, connecting team members with resources through the LiveWell platform, assisting with applications for assistance, and fostering relationships with community partners.

Responsibilities:

• Conduct needs assessment data analysis
• Utilize the LiveWell platform for resource connection
• Assist with application processes for team members
• Facilitate emergency care fund financial approvals
• Maintain relationships with community partners
• Communicate with department managers for support
• Conduct educational sessions on whole-person health

Requirements:

• Bachelor?s degree in Health Education
• MSW or Certified Health Coach preferred
• Two years' healthcare experience
• Effective verbal and written communication skills
• Ability to work varied shifts, including weekends and nights
• Strong organizational skills
