The position is responsible for conducting needs assessments, connecting team members with resources through the LiveWell platform, assisting with applications for assistance, and fostering relationships with community partners.
Responsibilities:
- Conduct needs assessment data analysis
- Utilize the LiveWell platform for resource connection
- Assist with application processes for team members
- Facilitate emergency care fund financial approvals
- Maintain relationships with community partners
- Communicate with department managers for support
- Conduct educational sessions on whole-person health
Requirements:
- Bachelor?s degree in Health Education
- MSW or Certified Health Coach preferred
- Two years' healthcare experience
- Effective verbal and written communication skills
- Ability to work varied shifts, including weekends and nights
- Strong organizational skills
