Responsibilities:

• Conduct and analyze needs assessment data
• Utilize a platform to connect individuals with resources
• Assist in applications for assistance and benefit enrollment
• Liaise with emergency care fund for financial approvals
• Build and maintain relationships with community partners
• Communicate with department managers for support
• Conduct educational sessions on whole-person health

Requirements:

• Bachelor?s degree, preferably in Health Education
• MSW or Certified Health Coach Preferred
• Two years? experience in healthcare
• Effective communication skills
• Ability to work under time constraints
